A Rehabilitation Assessment is provided following an injury or discharge from hospital.  It involves an in-home assessment to determine an individual’s functional abilities and the need for services, aids and therapy that will enable the individual to live safely and independently.

Following this assessment – ongoing case management can be provided to monitor the individual’s progress and ongoing need for services, aids and therapy.
